weekly accident and sickness benefits for Active Members

If you become Disabled from non-occupational bodily injury or sickness, the Plan will pay you a weekly benefit. This benefit is described in detail in the Health and Welfare Summary Plan Description (SPD) booklet.

How to Apply for Weekly Accident and Sickness Benefits

For the Fund to consider Weekly Accident and Sickness Benefits, you must submit a fully completed claim form. Both you and the Physician must complete the form. If possible, have your present Employer complete its portion of the claim form.
If you were laid-off at the time of disability, indicate this on your claim form.
The Fund must receive a Return to Work Notice completed by your Physician.
