Vision benefits

Vision benefits are available to all eligible Active and Disabled Employees, Retirees and their eligible Dependents. The Plan provides a wide range of vision care. These benefits are described in detail in the Health and Welfare Summary Plan Description (SPD) booklet.

Complete Examination	Plan pays 100% once each calendar year
Contact Lenses or Lenses and Frames	Plan pays 100% once each calendar year
Calendar Year Maximum	$200 per person
Prescription Safety Glasses (Members Only)	Plan pays up to $200 per Employee per calendar year (in addition to other Vision Benefits)
