Prescription Drug Benefits are available to all eligible Active and Disabled Employees, Retirees and their eligible Dependents through the Prescription Service Division of Caremark, Inc. These benefits are described in detail in the Health and Welfare Summary Plan Description (SPD) booklet.

There are many medications that produce the same results but differ significantly in cost. The Plan has developed a list of medications called a “formulary,” which consists of generic and brand name drugs that are as safe and effective as their alternatives, but cost less.
As you can see from the above schedule, you pay more when you purchase a brand name medication instead of a generic medication. Your costs increase even further when you take a brand name medication that is not on the formulary. So you should discuss the formulary with your doctor.
