Your dependents become eligible for coverage on the date you become eligible, or, if later, on the date they satisfy the Plan’s definition of an Eligible Dependent. This is explained in detail in the Health and Welfare Summary Plan Description (SPD) booklet.
Your dependent child(ren) may also be eligible for coverage if the Fund receives a Qualified Medical Child Support Order (QMCSO), which is a court order or an administrative order relating to child support providing for a child’s coverage under the Plan. A copy of the Plan’s QMCSO procedures is available, free of charge, upon request to the Fund Office.
